Effectiveness and safety of sutureless glue-free conjunctival autograft versus sutured conjunctival autograft in primary pterygium surgery: randomized controlled trial

Abstract

Background. This study aims to know if sutureless, glue-free conjunctival autograft comparable to sutured conjunctival autograft in primary pterygium surgery in terms of effectiveness.
Objective. To compare the effectiveness, safety, and surgical time of sutureless and glue free conjunctival autograft versus sutured conjunctival autograft in primary pterygium surgery.
Design. Randomized control trial study design.
Setting. Ophthalmology Outpatient Department Clinic of the Southern Philippines Medical Center, Davao City, from September 2014 to September 2015.
Participants. 84 patients diagnosed with primary pterygium, 25 to 70 years old.
Intervention. Participants were randomized to either sutured or sutureless glue-free conjunctival autograft in pterygium surgery.
Main Outcome Measure. Treatment effectiveness in terms of graft retention and recurrence rate, treatment safety in terms of visual deterioration and complications; and surgical time.
Main Results. The sutured group and sutureless group were comparable in terms of demographic and clinical profiles at baseline. Graft retention in the sutured group was (42/42; 100%) and in the sutureless group (39/42; 91.67%) (p=0.077). Recurrence rates within 3 months were 0/42 in the sutured group and 3/42 (7.14%) in the sutureless group (p=0.077). There was no visual deterioration observed among the participants. Foreign body sensation was observed in the sutured group 35 (83.33%) while there are 11 (26.19%) in the sutureless group. Mean surgical time of 25.34 minutes in the sutured group and 16.42 minutes in the sutureless group.
Conclusion. Sutureless glue-free conjunctival autograft did not significantly differ from sutured autograft in pterygium surgery in terms of safety and effectiveness. However, sutureless glue-free conjunctival autograft is faster and minimizes foreign body sensation because of the absence of sutures.
